I, the above named Player (or Parent of Player if he is under the age of 18), understand that participation in recreation programs can involve vigorous activity. I hereby assume all risk and hazards incidental to participation of the Player in the above mentioned program(s), including transportation to and from all activities and, do so hereby waive, release, absolve, indemnify and agree to hold harmless the Muslim Football League & Ontario Muslim Sports Organization, its officers, agents and employees, the coaches, the administration committee, and co-sponsors from any claims arising out of injury to the above named participant. In addition, I give my permission to hospitals and/or other licensed facilities to provide treatment as deemed necessary by them. I also agree to oblige to all the rules and decisions made by referees, coaches and captains. I understand that the commissioner’s word is final when it comes to trades, penalties, and suspensions. All Covid-19 protocols will be followed as set out by the Town of Aurora, the Province of Ontario and the Federal guidelines. I agree that I am vaccinated or will be vaccinated by October 17th, or have a valid medical exemption (or due to a valid religious creed that renders me exempt). I agree with all rules set out by OMSO and MFL Return to Sports policy.
